New Business Registration (Required for all businesses)
EIT Employer Questionnaire (Required only for businesses who also will remit Local Services Tax and/or Mercantile/Business Privilege Tax)
A separate registration is required for each location. Please complete one or both of the above forms to register a new business.
Maryland businesses making courtesy withholdings should list the Maryland work location on the registration.
Separate registrations should not be completed for each employee unless the employees are home based.
